經痛、腹痛、性行為時疼痛都是明顯的症狀。患者常常會問的問題就是朱古力瘤會不會破掉，因為囊腫內積血如果不治療會不斷增加，囊腫也因此跟著變大。當囊腔內積血過多、壓力過高時，可撐破囊壁薄弱部分，發生囊腫自發性破裂，囊內流出部分積血，壓力降低後，囊壁破口常可自行癒合。時間一久囊內積血再次增多、壓力再次增大時，又可發生自發性破裂。而激烈的性生活則會導致囊腫破裂，囊腫破裂後流至盆腔內的積血會刺激腹膜引起劇烈腹痛。
